Abstract
Around one in three individuals will develop cancer in their lifetime, and it is estimated that around 20 million European citizens will be cancer survivors. It is widely recognized in Europe that the current model of hospital-based cancer care has inadequate capacity and limited time to provide follow-up that meets endorsed survivorship practice guidelines and is not best suited to meet the chronic and complex survivorship needs of individuals and their families.
The INE-CSC project's main objective is to systematically support the sustained translation of evidence-based interventions into routine clinical practice as part of a cross-border cancer survivorship pathway that ultimately improves the health and well-being of cancer survivors.
This network will use an international comparative approach to map and create preliminary models that outline the contextual factors affecting: the implementation of cancer survivorship care and risk-stratified survivorship care pathways and the use of digital/electronic health solutions. Thus, an online platform will be created that includes an integrated theoretical framework based on implementation science and a toolkit to support the multi-level implementation of evidence-based cancer survivorship care across Europe.
